If this is your first visit to the studio or first visit for this calendar year, please fill out the CLIENT INTAKE form, as well as the corresponding forms for your selected services. 

If this is your first facial, please fill out the FACIAL TREATMENT form.  This will allow us to provide the safest and most beneficial services to you.  If you have previously filled one of these out, you will only need a RETURNING TREATMENT form.

If you are receiving any services with CBD products, you will need to fill out the CBD FACIAL waiver, as well as have a completed FACIAL TREATMENT form (Or RETURNING TREATMENT form) for each appointment we use these products. 

If this is your first time having a Microdermabrasion Facial, please fill out the MICRODERMABRASION TREATMENT form. If you have previously filled one of these out, you will only need a RETURNING TREATMENT form.

 If this is your first time having a Chemical Peel Facial,  please fill out the CHEMICAL PEEL TREATMENT form. If you have previously filled one of these out, you will only need a RETURNING TREATMENT form.
